ALW Assisted Living Waiver

Golden Age Advocacy helps families access California’s Assisted Living Waiver (ALW) Program — a Medi-Cal program that pays for the assisted living portion of care in a Residential Care Facility for the Elderly (RCFE) or an Adult Residential Facility (ARF).

How It Works

  • Medi-Cal pays for the assisted living services (care and support) at a licensed RCFE or ARF.

  • The resident is responsible for “room and board” (usually from their SSI income).

  • Families may choose from participating ALW facilities in approved California counties.

  • The program is designed to keep seniors in the community and out of nursing homes.

Who Qualifies?

You may qualify for the ALW if you:

 Are 65 or older (or under 65 with a disability).

 Have full-scope Medi-Cal with no share of cost.

 Need help with daily activities (bathing, dressing, meals, medications, etc.).

 Can safely live in an assisted living setting with the right supports.

 Would otherwise be at risk of living in a nursing home without this program.

Additional Eligibility Rules

  • Room & Board Payment: Residents must pay this portion directly to the facility, usually using their SSI income.

  • SSI with NMOHC (2025 rates):

    • SSI with the Non-Medical Out of Home Care bump-up = $1,599.07/month.

    • Resident keeps $179 for personal needs.

    • Facility receives $1,420.07 for room & board.

  • Important: Families or residents must be able to pay room & board. ALW does not cover this cost.

ALW vs. CalAIM

  • ALW is a long-established Medi-Cal waiver program that has a waitlist in most counties.

  • CalAIM is newer, offered through Managed Care Plans, and does not currently have a waitlist.
